Spring Nomination Form

Coaches,

We are asking for your help in telling us whom you think is a candidate for the spring season. 

Remember that most teams will only have two or at most three all-star players.

We want to know who your stars are?  Do they play offense, defense or a particular position well?   Do you have any great goalies? 
Do you have players on your team who have parents that referee? Do these kids come to practice and have a good attitude?

All these factors come into play when coaches choose their team.
Please fill out the information below and don’t forget to put in jersey #’s for those players that you want them to look at.
